Mission Statement
The Mission of Holy Family Sports Medicine is to provide our student-athletes with quality healthcare utilizing the most effective technologies and evidence-based practices available. The Holy Family Sports Medicine staff is dedicated to using a proactive approach to help minimize the risk of injuries. We are also committed to providing an environment that uses specialized rehabilitation processes to facilitate a return to play for injured student-athletes as soon as possible. In addition, the education of student-athletes regarding their health, safety and well-being is a priority
Pre-participation Paperwork
All student athletes are required to complete pre-participation paperwork before being eligible to participate in Holy Family Athletics. The components of the pre-participation paperwork differ for incoming/new student-athletes and returning student-athletes. The deadline to submit this paperwork is Friday, August 1st, 2025. All paperwork is completed via the ATS portal system. Hard copies and/or emailed copies of pre-participation paperwork will not be accepted.Â
Incoming Student-Athlete Requirements:
All freshmen and transfer student athletes will receive information from their head coach regarding the requirements for pre-participation paperwork. If you did not receive this information, a copy of the instructions can be found

Returning Student-Â Athletes Requirements
All returning student athletes will receive information from their team's athletic trainer to their HFU email address regarding the requirements for pre-participation paperwork. If you did not receive this information, please reach out to your team's athletic trainer.Â
Concussion ManagementÂ
The Holy Family University Department of Athletics has recognized that concussions pose a significant health risk for those students participating in athletics. With this in mind, the Department of Athletics, through the Sports Medicine staff, has implemented policies and procedures to assess and identify those student-athletes who have suffered a concussion.Â
Secondary Insurance InformationÂ
All Holy Family student-athletes are required to have their own primary insurance. Holy Family Athletics provides secondary insurance through AG Administrators for any injury that is sustained while participating in athletics. The sports medicine staff will file and submit claims, but it is the responsibility of the student-athlete to submit any and all necessary billing information directly to AG Administrators.Â

Visiting Team InformationÂ
The Holy Family Sports Medicine staff is happy to provide athletic training services to visiting student-athletes. Please note, for any services beyond taping and basic modalities (e.g. moist heat packs, ice bags), requests must be made via email in advance. The Athletic Training Room is located on the ground floor of the Campus Center building.
